Job Summary:

Signallamp Health is growing rapidly, and we are looking for motivated RNs and LPNs to work with us! Work remotely while caring for patients and helping them better manage their chronic conditions.

Responsibilities and Duties:

• Call your patients monthly and as needed.
• Establish a relationship and credibility with the patients and their families.
• Effectively communicate with patients’ physicians and broader care team.
• Ensure that patient is receiving the appropriate care guided by physician’s care plan and your experience.
• Provide ongoing education to help patient better manage their chronic illnesses.
• Create and update comprehensive care plans and care summaries.
• Document all communication and coordination of patient care.
• Follow all privacy policies and maintain the confidentiality of protected health information.

Qualifications and Skills:

• RN/LPN license in PA, NY and/or NJ
• Minimum of two years of nursing care to chronically ill patients or in home health or primary care setting.
• Bilingual in English/Spanish is a plus
• Electronic Medical Record (EMR) experience
• Demonstrates excellent active listening, written, verbal communication skills.
• Demonstrates excellent organizational and time management skills.
• Provides compassionate care.
• Possesses problem solving skills.
• Responds positively to change, showing willingness to learn new ways to accomplish tasks.
• Adheres to high standards of personal and professional conduct.

• Provide ongoing education to help patient better manage their chronic illnesses.

Qualifications and Skills

• RN/LPN license in any state
• Minimum of two years of nursing care to chronically ill patients or in home health or primary care setting.
• Bilingual in English/Spanish is a plus
• Electronic Medical Record (EMR) experience a must within the last 2 years.
• Strong computer skills, must be able to navigate through multiple application simultaneously.
• Demonstrates excellent active listening, written, verbal communication skills.
• Demonstrates excellent organizational and time management skills.
• Provides compassionate care.
• Ability to work independently under supervision and demonstrate problem solving skills.
