Job Summary:

Signallamp Health, Inc is currently seeking a Care Management Enrollment Specialist in Scranton, PA. The patient engagement specialist enrolls patients in a new care management program for patients with multiple chronic conditions.

Your primary duties include but are not limited to:

  • Making outbound calls to patients who are eligible to participate in their provider’s care
    management program.
  • Describing the benefits of the program and the reasons their primary care physician recommends enrolling
  • Encouraging patients to enroll in the program
  • Gathering and recording certain patient information in accordance with policies and procedures
  • Using call scripts to communicate all necessary information to patient
  • Meeting productivity metrics (with incentive bonus if minimum metrics exceeded)
  • Experience with Electronic Medical Records a plus

Responsibilities and Duties:

  • Call your patients monthly and as needed.
  • Establish a relationship and credibility with the patients and their families.
  • Effectively communicate with patients’ physicians and broader care team.
  • Ensure that patient is receiving the appropriate care guided by physician’s care plan and your experience.
  • Provide ongoing education to help patient better manage their chronic illnesses.
  • Create and update comprehensive care plans and care summaries.
  • Document all communication and coordination of patient care.
  • Follow all privacy policies and maintain the confidentiality of protected health information.

Qualifications and Skills:

  • RN/LPN license in PA, NY and/or NJ
  • Minimum of two years of nursing care to chronically ill patients or in home health or primary care setting.
  • Bilingual in English/Spanish is a plus
  • Electronic Medical Record (EMR) experience
  • Demonstrates excellent active listening, written, verbal communication skills.
  • Demonstrates excellent organizational and time management skills.
